 lish\, about the medical issues their person is most likely to face and th
 en how these conditions are treated. They then turn this knowledge into a

 ecisions.\n\n<h6>8 Week Program Goals</h6>\n<ul><li>Be able to confidently
  communicate your person’s goals with nurses\, doctors\, or anyone else ca
 ring for them.</li>\n<li>Know your person will receive the care they would
  want through understanding treatment options for the most common medical 
 issues that happen in dementia and aging.</li>\n<li>Have created a written
  decision-making plan (100% adaptable overtime) using Dr. Lamb’s process f
 or each medical issue you are likely to face.\nKnow if you come to the hos
 pital or ER and are forced to make a quick decision\, you have a plan and 
 knowledge to do so confidently.</li></ul>\n\nYou’ll go from feeling worrie
 d about the unknown\, to feeling informed and ready to act\, allowing you 
 to focus your time and attention on your person’s day-to-day needs and qua

 your role as medical decision maker</li>\n<li>Module 2 - Things to act on 
 and consider before creating a medical care plan</li>\n<li>Module 3 - Dete

 ing the plan - A process for checking in & a framework for decision making

 hen someone is seriously ill</li>\n<li>Module 8 - Tools you can use to adv

 m January 15th through March 10th. Each weekly module will open on Sunday 
 for participants to access as needed. Virtual Q&A session will be held at 
 the end of the week to review each module’s materials.\n\nAttendees will b
 e emailed directions to log-in after registering for the series. Optional 
 final meeting in March will be held at Insight - Fairfax.\n\nCost for the 
 full series is $250 per person. Dr. Lamb has partnered with Insight to pro
 vide scholarship for 10 people to attend for a reduced fee! \n\n<h6>Presen
